Service Delivery:

We assist individuals in gaining access to needed program and state plan services, as well as needed Medical, Social, Educational and Other services.

  • Provide comprehensive case management services designed around the participant and family’s needs.
  • We develop individualized service plans for the people we serve.
  • We help individuals and families access resources in the community.
  • We ensure that the services funded by DDD are utilized in accordance with the parameters defined in the supports program policies and procedures manual.
  • Together we advocate with you for Justice, Inclusion and full community participation.

Apex Compassion LLC helps individuals, and their families connect with appropriate DDD services, as well as other needed medical, social, and educational services.

Qualification

To Qualify For NJ Division of Developmental Disability Services(NJDDD):

  • You must be a New Jersey Resident
  • NJDDD and Medicaid eligible
  • Meet functional criteria determined by NJDDD
